It has to do with male anatomy and specifically the state of male arousal. The scrotum becomes tighter so the testicles, commonly referred to as balls, seem more prominent.

And then as the stages of arousal continue, the testicles grow until orgasm occurs after which the testicles and penis both return to their original state.

When you add into this the fact that male ejaculate is a white liquid and the spheres Red has disperse a white substance, a lot of innuendo was bound to happen.
